  • Teladoc Condition Management helps you take control of diabetes with support and supplies.
  • Thrive offers virtual physical therapy with licensed physical therapists to help you or a medical plan-enrolled family member move pain-free again.
  • Bloom provides digital pelvic health care for individuals with female anatomy intended to address pelvic conditions that occur throughout adulthood, pregnancy, postpartum, and menopause.

Teladoc Condition Management

Teladoc Condition Management helps you take control of your health condition if you're living with diabetes or hypertension. You'll receive easy-to-use technology and guidance from Teladoc Health – all at no additional cost to you.

If you and your family are enrolled in an AXA XL medical/pharmacy plan and you or a family member has pre-diabetes, diabetes, or hypertension you can participate in this program.

Depending on your condition, you'll receive the following:

  • For patients with pre-diabetes or diabetes: You'll receive an advanced blood glucose meter that transmits your blood glucose readings to your phone, in addition to free and ongoing test strips delivered right to your door.
  • For patients with hypertension: You'll receive a connected blood pressure device that transmits your blood pressure readings to your phone.

In addition to the smart devices, you'll also have access to the Teladoc Health app, where you can get in-depth insights on your readings and one-on-one support from expert health coaches.

Personalized pelvic care through Bloom

One in four women suffers from moderate to severe pelvic health disorders, but many do not seek care for their pain or discomfort. Whether the pelvic disorder originated from childbirth, menopause, surgery, or other causes, care and support are available.

Bloom is one such solution intended to address pelvic health disorders. Featuring a unique, FDA-listed intravaginal device, the Bloom Pod, and a smartphone based app, Bloom combines tailored physical therapy with digital tracking and education.

Through the program, you’ll choose a Pelvic Health Specialist and set a training plan that includes pelvic-floor and pelvic-girdle muscle exercises that use the Pod. The Pod measures the contraction and elongation of the pelvic floor, and the Pod reports these measurements to your Bloom app for your Pelvic Health Specialist to review and incorporate into your treatment plan. Your data is confidential, and Bloom’s digital platform allows you to receive your pelvic care from the comfort and privacy of your own home.

  • Bowel health
  • Pregnancy
  • Postpartum
  • Menopause

Virtual physical therapy through Thrive

Physical pain and decreased mobility isn’t just uncomfortable— it can be mentally taxing and discouraging. Add on the difficulty of scheduling and traveling to physical therapy appointments, and physical pain suddenly becomes even more frustrating.

Through AXA XL’s partnership with Thrive​, all colleagues and their AXA XL medical plan-enrolled dependents ages 13 and up can now receive free virtual physical therapy sessions with licensed physical therapists from the comfort of your home.

Through Thrive:

  • Participating individuals are matched with a licensed physical therapist. After a comprehensive evaluation, your therapist develops an individualized plan designed around your diagnosis, abilities, and goals.
  • You’ll receive tailored care for any musculoskeletal issues affecting you, from neck pain to ankle sprains. Your physical therapist will help you work toward your goals, whether that means living pain medication free or being able to get outdoors and bike with your kids.
  • Your movement program is integrated into the digital app and website so you can practice your exercises anytime, anywhere. Thrive also provides wearable sensors that work with Thrive’s Digital Therapist to provide you with real-time feedback on your exercise movements. The Digital Therapist provides data for your physical therapist to analyze ahead of your next check-in.
  • You can access videos about musculoskeletal health or quick flexibility exercises, chat with your physical therapist, and even learn behavioral therapy techniques that help you work through your pain all from the ease of the Thrive app.

Thrive is not just for those in immediate physical pain either. It can help anyone maintain their physical health, avoid injuries, and stay active. Whether you’re an athlete, recovering from surgery, or just want to move more, Thrive’s targeted exercises can help you improve your strength and mobility. Thrive is not intended as a total replacement for in-person physical therapy, but can help you feel and move your best.

Anyone can take advantage of Thrive at any point throughout the year. You don’t need to enroll during Annual Enrollment to start using Thrive. It’s important to note that in-person physical therapy continues to be offered through the AXA XL medical plan. Visit Thrive​ to get started.

A cost-saver in the long-run

We know physical pain can be a real obstacle to feeling your best, but the cost of in-person therapy sessions can add up both from a dollar and time perspective. From our employee focus groups, we learned that’s why our free Thrive physical therapy program is so valued among colleagues. For some colleagues, Thrive has even proved so useful as to eliminate the need for intensive surgery!

Fitness and movement support with Move

Even if you're not in pain now, actively improving your fitness and mobility can help prevent pain down the line. With Sword Move, you can receive dedicated support from a personal trainer, wearable devices, and personalized movement programs to help you prevent musculoskeletal issues.

Move offers:

  • Personal training: A committed and certified personal trainer will accompany you throughout your fitness journey.
  • Wearable technology integration: Sync a wearable device to track your progress and gain insights into how factors like your environment, sleep patterns, and daily habits influence your health. This data will be shared with your personal trainer, enabling them to customize your program to your needs.
  • Tailored exercises: Eliminate the uncertainty and intimidation of starting a new fitness routine with personalized exercise plans complete with video tutorials. Each week, you'll access a set of movement modules designed around your objectives, lifestyle, progress, and availability.

Move is available to all colleagues and their AXA XL medical plan-enrolled dependents ages 13 and up, and users can sign up at any point throughout the year.
